Adiposity, Metabolic Parameters and Circulating Leptin

Abstract

Nowadays most women are interested in their body shape and image, so many beauty-related products were developed and increasing as time goes by. Hence the topic in medical practice field are changing. Long long times ago, human's will to solve the obesity were going on. But the key to do so was not newer than before. In 1994, a young investigator named J Friedman in Rockefeller university found the new molecule(leptin) related with obesity. Most bariatric physician in general practice usually check the basic blood profile, bodyfat, E K G and sonography based on American Society of Bariatric Physician criteria to know the metabolic state on adiposity. In adiposity, so many markers expressed in adiposity were known such as %bodyfat, BMI, Percent Ideal Body Weight. So far nobody knows which is well-expressed marker to associate with the metabolic factor.
The aim of this study is to examine the association among the leptin, the metabolic variable and adiposity. And to compare the leptin, metabolic variable(FBS, FFA, Total Cholesterol, Triglyceride, HDL Cholesterol, TSH, Uric Acid) on adiposity(BMI, PIBW, %bodyfat) in overweight women.
30 overweight women(bodyfat>26%) visited in charm clinic were included. Their age and BMI were 31.3±8.48 years old, 26.02±3.24, respectively. Initial% bodyfat, PIBW(Modified Broca) were 34.75±4.12%, 125.59±16.18%, respectively. The initial leptin was 14.51±75ng/mL.
The serum leptin level was positively associated with % bodyfat(r=0.497**), BMI(r=0.697**), PIBW(r=0.643**), respectively. The serum leptin level was positively associated with triglyceride(r=0.462*) in metabolic variables, so do other adiposity markers as BMI(r=0.499**), PIBW(r=0.451*), bodyfat(r=0.449*). After age and uric acid were controlled, triglyceride in metabolic variables was significantly positively associated with BMI(r=0.572**), PIBW(r=0.5438**), %bodyfat(r=0.4556*), leptin(r=0.5872*), whereas HDL-Cholesterol was significantly negatively associated with leptin(r=0.4775*), BMI(r=0.4309*), PIBW(r=0.4246*).
After data were devided into two group(obese, non-obese), the metabolic variables and serum leptin on adiposity as PIBW, BMI, %bodyfat were compared. Triglyceride in obese group(BMI>= 26) was significantly higher than that in non-obese group(BMI<26) based on BMI*. Total cholesterol in obese group(bodyfat>=30%) was significantly higher than that in non-obese group(bodyfat>=30%) based on bodyfat**. The serum leptin in obese group was significantly higher than that of non-obese group on adiposity(PIBW**, BMI**, % bodyfat*).
This study showed that triglyceride and leotin were significantly associated with adiposity. Because it is suggested that most of subject like to have carbohydrate. More larger data about leptin will be recruited in Korea. Still neither adiposity such as PIBW, BMI, % bodyfat does seem to express metabolic state well.
